Membership Application Form 2008

If you were an ESGO member in the past you do not need to go through the whole application process, please click here for the call for dues form

Please note that according to the Bylaws of the Society, all new applications will be submitted for final acceptation during the next ESGO Business meeting which will take place during ESGO16 in Belgrade.

Before you begin, please prepare the following information:

1. Applicant full demographic details
2. Payment Details
3. If you apply for trainee category, a letter from head of department is required with the application.

ESGO MEMBERSHIP CATEGORIES

Kindly choose the type of your membership:

Membership categories (Active and Associate membership) defined in Bylaws are based on the country of residence and professional status. Furthermore you can choose an option to subscribe to the International Journal of Gynecologic Cancer (IJGC).

Please note that your membership period starts January 1^st, 2008 and ends
December 31, 2008

ACTIVE MEMBERSHIP
	€150	Individual clinicians or scientists, citizens of Europe (incl. Israel) (online and print copy of the IJGC incl)
	€100	Members (citizens of Europe) from Upper-Middle or Lower-Middle Economies (online and print copy of the IJGC incl) Eligible countries: Group A
	€25	Members (citizens of Europe) from Lower-Middle Economies (on-line IJGC incl) Eligible countries: Group B
	€75	Members of IGCS (citizens of Europe) that wish to receive the Journal via IGCS (no subscription to the IJGC)
ASSOCIATE MEMBERSHIP
	€150	Individual clinicians or scientists, citizens of non European countries (online and print copy of the IJGC incl)
	€75	Members of IGCS (non-European citizens) that wish to receive the Journal via IGCS (no subscription to the IJGC)
	€25	Trainee (on-line IJGC incl) - please provide a proof of training status by using the Trainees Declaration form
	€25	Nurses and non-MD professionals (on-line IJGC incl)
The category Trainees and non-MDs do not include subscription to the paper copy of the IJGC. If you wish to have the subscription to the journal choose the membership category according to your country of residence.
