You can also print the form, complete and return to Registration Department as soon as possible in order to benefit from the lower registration fees.

Payment:
Payment of registration fees may be made as follows:
- by credit card i.e. Visa, Mastercard, Diners Club or American Express.
- by cheque payable to ESGO13 Meeting.
- by bank transfer to ESGO13 Credit Suisse Bank Geneva Branch, Switzerland, Account number: 4251- 465953-82-21, swift code: CRESCHZZ 12A
Bank charges are the responsibility of the payee and should be paid at source in addition to the registration fees. Please make sure that the name of the meeting and of the participant is stated on the bank transfer.

Cancellation Policy
Notice of cancellation should be sent in writing to the Meeting secretariat. Cancellation received before January 31, 2003, will be subject to an administration fee of Euro 60.00. For cancellations received after January 31, 2003, 50% of the full applicable registration fee will be charged. Please note that all refunds will be made after the Meeting.
